چکیده
Many believe that the "next generation" of health promotion or health management programs will likely include a much more extensive alignment of environmental ['actors that support a healthy and productive workforce. To address this possibility, a model was conceptualized that integrates an environmentally-based intervention within a comprehensive health management effort. Intervention is direded at both enhancements to the physical aspects of the worksite as well as its cultural makeup, and a series of suggestions are provided for those who want to utilize this model. This comprehensive, environmentally-focused component of the next generation of health promotion programs will expand the population reach of health management to all employees and potentially enhance its overall success. This approach is viewed as critical in creating a healthy and productive work organization, and allows the employer to capture the full economic and human capital benefits of intervention. The creation of a "supportive environment" for health promotion and wellness is seen as an important develop ment in the field of worksite health promotion, thus our focus on this model within The Art of Health Promotion. In this edition we will examine the following topics:

Repolarization Alternans and Ventricular Arrhythmia in a Repaired Tetralogy of Fallot Animal Model
BACKGROUND Ventricular arrhythmia is an important cause of late death in patients with repaired tetralogy of Fallot (rTOF). By using an rTOF canine model, we investigated the role of repolarization alternans and its electrophysiological mechanisms. METHODS AND RESULTS Six dogs received right ventricular outflow tract (RVOT) transannular patch, pulmonary valve destruction, and right bundle bra...
متن کاملImplication of atrial size and function in adults with repaired Tetralogy of Fallot and the effect of treatment by ramipril: the APPROPRIATE randomised trial
Background Background: Patients with repaired tetralogy of Fallot (rtoF) are at risk of supra-ventricular tachycardia due to atrial dilatation. To date, the impact of atrial size and function has been poorly studied in this population. Angiotensin-converting enzyme (ACE ) inhibitor are known for their beneficial effects on atrial and ventricular remodelling in patients with cardiomyopathy. A pr...
متن کاملProarrhythmic remodelling of the right ventricle in a porcine model of repaired tetralogy of Fallot
OBJECTIVE The growing adult population with surgically corrected tetralogy of Fallot (TOF) is at risk of arrhythmias and sudden cardiac death. We sought to investigate the contribution of right ventricular (RV) structural and electrophysiological remodelling to arrhythmia generation in a preclinical animal model of repaired TOF (rTOF). METHODS AND RESULTS Pigs mimicking rTOF underwent cardiac...
متن کاملRelation of increased BMI to left atrial volume in repaired tetralogy of Fallot patients
Background Contemporary care of repaired tetralogy of Fallot (rTOF) patients is directed toward detection and avoidance of long-term morbidity and mortality, of which arrhythmias play an important role. Increased arrhythmia risk in the rTOF population has been associated with ventricular dysfunction and chamber dilation. In adults without congenital heart disease, obesity has been related to le...
متن کاملAssociation of ventricular dyssynchrony and strain with cardiac function in patients with repaired tetralogy of Fallot
Background Patients with repaired tetralogy of Fallot (rTOF) have impairments in cardiac mechanics (strain and dyssynchrony). However, the relationship between cardiac strain, dyssynchrony, and cardiac function (measured by ejection fraction) in patients with rTOF has not been explored comprehensively in a large population. We hypothesized that measures of cardiac mechanics are associated with ...
